This and a related pilgrim's badge (60.179.2) display the emblems commonly associated with devotion to the Immaculate Conception. One bears the monogram of the Virgin, while the other features an image of the monstrance of the Eucharist. Both probably framed devotional images like that of another venera (30.135.91) shown here as well.
